Conestoga Rugby Club requires that all players have their forms, paperwork, and dues into the club prior to participating in any practice or game. A confirmation email from the club's administration indicating that a player is approved is absolutely required for that player to participate in practice in any way. Players can attend practices and observe the action prior to being "cleared to play", but you're encouraged to get the paperwork completed as soon as possible to avoid any delay in being able to practice. - Read the Instructions Carefully.
- Complete the Conestoga Rugby Club Registration Form
- Complete the "PIAA Comprehensive Initial Pre-Participation Physical Evaluation" forms (You may be required to get a physical from your doctor in order to complete this. Please make sure you leave adequate time to get that appointment and return the forms.)
- Complete & Sign the T/E Assumption of Risk
- Complete & Sign the Parent/Student Acknowledgement of Guidelines forms
- Complete & Sign the EPRU Players Code of Conduct
- Complete the USA Rugby Participation Agreement & Waivers (DO NOT SEND THESE TO USA RUGBY DIRECTLY)
- Send all of the completed forms, and a check for $160 ($100 for each additional player from the same family) to the address below
- The club will send you an email which confirms that you're eligible to practice and play
